Community Support Professional

Qualifications:

  1. Must possess at least a bachelor’s degree from an accredited educational institution in the field of psychology, counseling, social work or a related field.  
  2. Equivalent experience may be substituted on the basis of one (1) year of experience for each year of required education training (for Community Support Services only).

Duties:

  1. Conduct individual rehabilitative sessions at the office, in client’s home or in the community.
  2. Participate in the development and implementation of appropriate treatment plans.
  3. Maintain an accurate and updated file on each client.
  4. Submit appropriate records for billing.
  5. Refer clients to community resources for financial, vocational, nutritional, medical, spiritual, transportation, and housing assistance.
  6. Comply with all laws, regulations and policies concerning the treatment of clients.
  7. Serve on committees as appointed.
  8. Provide billable services appropriate to the clients’ needs while meeting the production expectation for the position.
  9. Facilitate and participate in continuity of care of clients served. 
  10. Other duties as assigned.

Reporting Supervisor:

The Community Support Professional—shall be directly responsible to the assigned Community Support Supervisor.

Family Support Provider

Qualifications:

  1. A family member with a child who has been or currently has a behavioral or emotional disorder.  
  2. Missouri resident for at least two years.
  3. High school diploma or GED.
  4. Valid driver’s license 
  5. Must complete and pass the FSP Relias training and DBH FSP training

Duties:

  1. Provide information and support to the parent/caregiver so they have a better understanding of their child/youth’s needs and explore options to be considered as part of their treatment.
  2. Assist the parent/caregiver in understanding the planning process and the importance of their voice in the development and implementation of the treatment plan.
  3. Assist the family in understanding the roles of the various providers and the importance of “team” approach.
  4. Discuss the benefit of natural supports within their family and community.
  5. Introduce methods for problem solving and developing strategies to address issues that need work.
  6. Provide support and information to parents/caregivers of Transitional Age Youth related to the shift from being the decision maker to being the support to the youth as they become more independent.
  7. Connecting families to community resources. 
  8.  Provides appropriate documentation and billing as needed on family support services. 
  9. Comply with all laws, regulations and policies concerning the treatment of clients/family.
  10. Provide billable services appropriate to the family’s needs while meeting the production expectation for the position.
  11. Other duties as assigned.

The Family Support Provider—shall be directly responsible to the assigned Family Support Supervisor.

ITCD (Integrated Treatment Of Co-Occuring Disorders) Specialist

Qualifications:

  1. Must possess at least a Bachelor’s degree from an accredited educational institution in the field of psychology, counseling, social work or related field.
  2. Must also possess certification of Qualified Addiction Professional (QAP): CADC, CRADC, CRRAADC, CCJP, CCDP, CCDP-D, RADC, or RADC-P.

Duties:

  1. Will provide direct services to adult clients that are experiencing moderate to severe co-occurring disorders in CPR Program. These services will include individual and group co-occurring counseling as well as education. 
  2. Will provide screening and assessment of substance use.  
  3. Will conduct training sessions for the staff members of Bootheel Counseling Services.
  4. Other duties as assigned. 

Reporting Supervisor:

The ITCD Specialist will report to the CPRP Director.

Healthcare Home Nurse Care Manager (LPN or RN)

Qualifications:

  1. Must possess at least a Missouri LPN or RN License.
  2. Experience in a Health Care setting, such as hospital, clinic, or home health delivery preferred.
  3. Experience with Excel and Word programs.

Duties:

  1. Perform job duties based on Healthcare Home principles in accordance with the CMHC’s vision, mission, and values and participate in required HCH trainings and agency trainings.
  2. Provide care for consumers on their caseload.
  3. Participate in annual treatment planning including:
    1. Communicating face-to-face with consumers identifying health concerns and treatment goals and client’s desire to participate in Healthcare Home.
    2. Provide training on chronic diseases, health coaching, healthy lifestyle choices and medications.
    3. Reviewing and signing off on health assessment and treatment plan.
    4. Communicate with Community Support Professionals about client’s identified concerns and goals.
  4. Review client records and patient history within 90 days of enrollment.
  5. Coordinate care with external health care providers (pharmacies, PCPs, FQHC’s etc.)
  6. Document individual client care and coordination.
  7. Maintain a monthly HCH spreadsheet of their current caseload and track the required health assessments, metabolic screenings, and physician status for these consumers.
  8. Monitor HIT tools and reports for treatment and medication alerts, and hospital admissions/discharges and alerts Community Support Professionals as appropriate.
  9. Contact hospitals regarding:
    1. client admission
    2. Identify client as a member of Bootheel Counseling Service’s Healthcare Home  
    3. Request discharge orders and discharge summary to be faxed upon discharge and/or completion to Healthcare Home
    4. Contact clients within 72 hours of hospital discharge to conduct hospital follow-up and medication reconciliation.  
  10.  Provide training and educational groups regarding health, wellness, chronic disease, and health and wellness opportunities for staff and consumers to maintain healthy lifestyles and manage chronic diseases.
  11. Other duties as assigned.

Medication Clinic LPN

Qualifications:

  1. Must possess at least a Missouri LPN

Duties:

  1. Be assigned to a staff psychiatrist/nurse practitioner and complete duties as assigned by the prescriber. 
  2. Perform vitals and other needed procedures to check clients in for prescriber.
  3. Call in Prior Authorizations for medications as prescribed by prescriber. 
  4. Package, log and dispense medications and explain dosages to clients as needed.
  5. Complete intra-agency referral form, status reports, and other documentation requirements as assigned.
  6. Provide billable services appropriate to the clients’ needs while meeting the production expectation for the position. 
  7. Submit appropriate records for billing.
  8. Comply with all laws, regulations and policies concerning the treatment of clients.
  9. Other duties as assigned. 

Reporting Supervisor:

The Medication Clinic LPN shall be directly responsible to the Medication Clinic Coordinator

Staff Psychiatrist

Qualifications:

  1. Must be licensed to practice medicine in the State of Missouri.
  2. Must have completed a residency in psychiatry.
  3. Must be board-eligible.

Duties:

The Staff Psychiatrist shall:

  1. Provide psychiatric input to the Center in the areas of:
    1. Multi-disciplinary case staffing;
    2. Quality assurance;
    3. In-service training.
  2. Provide psychiatric evaluations, medical evaluations and medication check services at assigned outpatient/contract sites.
  3. Provide psychiatric management (defined as development of treatment plan, consultation with clinicians  and other staff, and direct face-to-face contact with patients).
  4. Consult with individual staff professionals including team participation, as needed.
  5. Advise Executive Director on psychiatric issues, programs and concerns.
  6. Provide a positive linkage to the general medical community in the Service Area.
  7. Exhibits consistency in job performance.

      CPRP Assessor

      Qualifications:

      1. Must possess a master’s degree from an accredited educational institution in the field of social work, psychology, counseling or related field.
      2. Must be recognized as a Qualified Mental Health Professional by the Missouri Department of Mental Health.

      Duties:

      1. Will provide CPRP assessments as assigned.
      2. Will work closely with CPRP staff, specifically Community Support Professionals and CS Supervisors regarding assessments.
      3. Participate in the development of treatment plans for annual assessments.
      1. Complete the scheduling of annual assessments with Community Support Professionals.
      2. Perform social histories/clinical interviews for intake and annual evaluation services.
      3. Complete all documents in a timely manner within guidelines of DMH and Bootheel Behavioral Health standards.
      4. Route all clinical documents for signatures and staff clinical changes with treatment providers.
      5. Submit appropriate records for billing.
      6. Other duties as assigned.

      Reporting Supervisor:

      The CPRP Assessor shall be directly responsible to the CPRP Assessment Coordinator.

      Clinical Therapist

      Qualifications:

      1. Must possess at least a master’s degree from an accredited educational institution in the field of psychology, social work, counseling, or other related field from an accredited university.
      2. Must have completed a practicum in behavioral health services.
      3. Must be eligible for licensure in Social Work or Counseling in the State of Missouri.
      4. Preference shall be given to those with additional graduate training and/or experience.

      Duties:

      1. Provide initial intake assessment and develop and implement appropriate treatment planning.
      2. Conducting ongoing individual, group, and marital and family therapy sessions.
      3. Provide crisis intervention.
      4. Participate in diagnostic and cooperative treatment team planning of appropriate cases.
      5. Responsible for maintenance of client files in accordance with all local, state and federal regulations.
      6. Submit appropriate records for billing.
      7. Provide billable services appropriate to the clients’ needs while meeting the production expectation for the position.
      8. Facilitate and participate in continuity of care of clients served.
      9. Participate in on-call rotation of MOCARS for this agency’s service area.
      10. Other duties as assigned.

      Reporting Supervisor:

      Each clinical therapist shall be directly responsible to the Clinical Director.

      Crisis Provider

      Qualifications:

      1. Must possess at least a master’s degree from an accredited educational institution in the field of psychology, counseling, social work or related field, or meet the qualification of a community support specialist with at least three (3) years of population-specific experience. Primary call responsibilities.
      2. Licensure preferred, provisional or eligibility for licensure is required.
      3. Preference shall be given to individuals with experience in delivery of crisis intervention services and mobile response.
      4. Possess strong skills in assessment, crisis intervention, and care coordination of individuals experiencing mental health emergencies.
      5. Ability to work a flexible schedule including evenings and weekends.
      6.  

      Duties:

      1. Provide assessment, crisis intervention, care coordination, treatment planning, and group work for individuals experiencing a mental health emergency.
      2. Work closely with other crisis providers, coworkers, community members, and family members in assisting with individuals experiencing mental health emergencies.
      3. Assist in the development and implementation of the agency’s urgent and crisis services.
      4. Provide primary on-call availability during and after agency’s regular business hours.
      5. Implement, assess and further develop crisis services consistent with the philosophy of DMH statewide ACI system.
      6. Manage referrals to crisis services effectively and efficiently.
      7. Educate consumers and other stakeholders of use and access of crisis cervices.
      8. Manage and coordinate agency MOCARS paperwork (crisis notes, etc.)
      9. Review, address and manage reports from BHR daily.
      10. Assist Coordinator of Crisis Services in orientation/training of new staff that will be providing on-call availability and crisis assessment and intervention.
      11. Coordinate services of high risk consumers such as hospital discharges.
      12. Maintain accurate and current client records.
      13. Participate in staff meetings.
      14. Facilitate agency staff with assessment and hospitalization procedures, as needed.
      15. Assist Crisis Services Coordinator in gathering/tracking statistical information related to crisis services and its’ outcomes.
      16. Other duties as assigned.

      Reporting Supervisor:

      The Crisis Provider shall be directly responsible to the Crisis Services Coordinator.
